Patients of all ages in need of post-acute care, rehabilitation services or disease management may be appropriate for home care services.

Most major insurance carriers, including Medicare, pay for home care services.
What are the eligibility requirements to be covered for home care services?
You must have a physical need that requires a nurse or therapist.
Do I need to have been admitted to the hospital before I can qualify for home care services?
No. There is no in-patient hospital stay required to be eligible for home care services.
Do I have to be homebound to be eligible for home care services?
Medicare requires a person to be “homebound.” This may be a temporary condition. By Medicare definition you may be considered homebound if the following two criteria are met:
1. Because of illness or injury, you need the aid of supportive devices such as crutches, canes, wheelchairs and walkers; the use of special transportation; or the assistance of another person in order to leave your place of residence, or you have a condition such that leaving your home is nor recommended by your doctor.
2. There must exist a normal inability to leave your home and leaving home requires a considerable taxing effort.
Do I need an order from my doctor to have home care?Physician orders are required to begin home care services, but patients, caregivers, family members, friends or medical staff personnel may request services. Our staff will help with getting an order from a physician.
