Discharge Planning
Planning for discharge will begin either before your admission or early in your stay to make sure that your transition from the hospital to home or another health care facility is as smooth as possible. Please be aware that hospital stays nationwide average only a few days. The decision for discharge is often not made until the day you leave, based on your condition and the level of care required, so timely planning is essential.
Your physician and primary nurse will discuss your discharge plan with you. If continuing services are medically necessary after you leave the hospital, a nurse case manager or social worker will assist you in making arrangements. We ask that you be prepared to leave the hospital by 11:00 AM on the day of discharge. If you have difficulty arranging transportation, please inform your nurse as soon as possible.
If you have concerns about your transition from the hospital, please let your physician or primary nurse know. You may also contact clinical resource management at 210-7441 to speak with a nurse case manager.


Nutrition Counseling
A registered dietician is available for those seeking a nutritional consultation. An individual session with the dietician can be arranged by simply speaking to your doctor who will refer you and advise you of any preparation or necessary paperwork. During nutrition consultation, patients receive information about calorie and fluid requirements, meals and snacks and even shopping tips.

Speech Therapy
At New Milford Hospital's speech-language pathology center, we understand how important the ability to communicate is… and how devastating it is when something interferes with that ability.
We are dedicated to providing you with the highest standard of care using state-of-the-art evaluation and treatment procedures. Our speech-language pathologists hold the certificate of clinical competence (CCC) through the American Speech-Language-Hearing Association (ASHA) and are licensed by the state of Connecticut. We see patients of all age ranges from infant to geriatric.
We accept most medical insurances and are happy to help you obtain the necessary prescription and authorization.

Extensive knowledge in language development and early intervention, pediatric and neonatal feeding and swallowing, children with multiple disabilities, augmentative/alternative communication, and passy-muir speaking valve assessment for patients with a tracheostomy.


Physical Therapy
The physical therapy department at New Milford Hospital offers a wide range of in-patient physical therapy services. Treatment diagnoses include therapy for:
- orthopedic surgery recovery, fractures or amputation,
- stroke, neurological disorders,
- pulmonary dysfunctions,
- burns/wounds and
- other medical diagnoses resulting in decreased physical functioning.
The rehabilitation department's goal is to provide quality and compassionate care to all of our patients.
- Within 24 hours of a physician referral, a comprehensive, initial evaluation and plan of care that incorporates patient and family-oriented goals, is implemented.
- Treatment includes training and education for gait and stairs, functional mobility, coordination and balance, strengthening exercises, as well as extensive education for patient and family members.
The physical therapy department works closely with an interdisciplinary team, including physicians, nurses, case managers and social workers, as well as other therapy departments. This comprehensive approach works to ensure a safe, therapeutically progressive in-patient stay, as well as an appropriate and safe discharge plan.


Cardiac Rehabilitation and Supervised Fitness
New Milford Hospital offers cardiac rehabilitation and supervised adult fitness programs. These services are provided by a health care team of licensed professionals, including physical therapists, cardiac rehabilitation nurse specialists and, in the case of our cardiovascular disease prevention program, an exercise physiologist.
Together they address an individual's needs with a comprehensive, medically sound approach. A physician's referral is required to receive therapy and participate in the fitness program.
The center is centrally located at The Club, 130A Grove Street in New Milford. Convenient, safe parking is located behind The Club.
