Western Connecticut Home Care Honors Providers
DANBURY, Connecticut – November 4, 2016 – During the month of November Western Connecticut Home Care joins the National Association for Home Care & Hospice (NAHCH) in honoring the nurses, therapists, social workers, and home health aides across the country that provide healthcare at home to over 5 million patients in the U.S. annually.
Home care services have existed for well over 100 years. Yet never has it played a more vital role in the healthcare continuum than it does today. As payment models evolve and as we face the onset of an expected 20 year growth in the aging population, payors and healthcare systems are shifting to home and community-based care to help reduce healthcare costs.
Home care allows patients of all ages to receive a wide variety of health care services in the comfort of their homes by qualified professionals and para-professionals. With the aim of reducing hospitalizations and promoting disease self-management, home care provides person-centered delivery of care focusing on the patient and family.
Western Connecticut Home Care (WCHC) is meeting the ever growing need of home care by hiring qualified, experienced staff to ensure adequate clinician to patient ratios, and using technology for clinical documentation and remote patient monitoring for more efficient delivery of care. Our affiliation with Western Connecticut Health Network allows a seamless transition of care from acute or sub-acute settings to home.
Specialty trained clinicians provide a variety of services, 7 days a week, that include: medication management; chronic disease care and management; wound care; infusion therapy; pain management; fall assessment and management; complex medical issues; and physical, occupational and speech therapies for post-orthopedic surgery including joint replacement, gait training and lost function of ADLs.
